What the Season 15 records hold, and the steps where they carry them.
Read out of the Diablo IV 3.2.1 game data and checked against the Season 14 build, so “new this patch” means the record itself is new. Drop tables and server scripts are not in that export. Where a chain runs into one, the steps stop and say so.
Two worlds, both records new in 3.2.1:
The ??? Nightmare Sigil people are finding is the door. Its record is DungeonKey_SecretLevel, and the keyed-dungeon table gives it one destination:

Three side legs hang off the chain, each gated on a choice from the Fractured Peaks prologue. Anger opens an Amazon leg with an Unusual Key and The Training Grounds, reached from Temis. Pride opens a jail leg with a corrupted Ballista, an Amazonian Bolt to load it, and N’Facl, the Jailer of Angels, in Idyllic Reach. Greed opens the ritual in the Forlorn Cellar.
A sin can be handed over: “A passing wanderer has bellowed at you. You feel the weight of a new sin.” So you can reach a leg you did not choose.
The Map of the Stars is a new quest item. Used anywhere it does not belong: “You can’t use this now, or here.”
What it opens is new this patch: a world named ???, “Like a lamb to the slaughter.” Its areas are the Temple of Baaaaaaal and the Shepherd’s Conclave.
The ritual and the place you use the map are both server-side.
New world this patch. Boss room the Den of Whimsy, boss Neigh’relle, quest “Sparkling Happiness and Rainbows”.
Chapter 5’s own text is about clearing Worldstone Keep and never mentions the potion.
Inside: Unicorns, Cuddle Bears, Twinkleroot, Happy Clouds, Candy Jars, Armor and Weapon Gifts, and chests called Soup and Resplendent Soup. Two named unicorns, Donut and Lore-ass.
The Pit’s row in the keyed-dungeon table lists three entries that are not tilesets:
All three print as The Pit with the ordinary subtitle and a Pit Depth number, so the loading screen gives nothing away.
Moophisto, the Divine Bovine, is a world boss. His arena is the Bovine Sanctum, which has its own waypoint between Kurast Docks and Gea Kul.
New this season: an Altar of the Heifer with a summoning quest attached, plus a Bovine Blacksmith, Jeweler and Occultist.
Which cache holds which piece is not in the files.
Also here: a Bell of the Bovine amulet wired to a moo emote, and an elixir called Udder’s Milk.
DClone is in Season 15, filed under ambush. The Prime Evil Ambush system is new this patch with a leg for each of the three, and Diablo is the only one with an uber form: Echo of Uber Diablo, flavour name The Dark Lord. Two lines exist for it:

What makes it proc, and what makes it roll uber, are server-side.
Both are new this patch and they take different slots. Annihilus is a Horadric Seal. Hellfire Torch is a Charm. So they can be worn together.

The Torch’s fourth line is the attribute Unique_Charm_Count_Bonus at 1: wearing it lets you equip one more unique charm. There is also a set of per-class Torch affixes that each read +1 Set count to a named Talisman set, five per class, so a Torch can push a set one tier up.
Neither has a grant in the export. In Diablo II the Torch came from Uber Tristram and Annihilus from Uber Diablo, and this season has both, but no record joins them.
The Stone of Jordan is new too, as a talisman charm with two cube recipes: “Your Resistances are each equal to your highest, and you deal more damage of that Element.”
All new this patch. The world World_S15_FesteringNest prints as Diablo Hellfire - Lester’s Farm. A Journey achievement, Na-Krul and Unusual, reads: “Complete Diablo Hellfire: Lester’s Farm in Torment V or higher.” A second criterion names Diablo Hellfire: Festering Night.
The boss is Na-Krul, The Lord of Hellfire. The game places a Door, a Lever, three Spellbooks and Hell’s Fingers around him. Each book carries one line:

The order is in the game too. Every Splinter of Hellfire carries the three as one line of flavour text:
“In Spiritu Sanctum. Praedictum Otium. Efficio Obitus Ut Inimicus.”
Whether the lever opens the door with him at full strength, as it did in Hellfire, is a server script and not in the files.
Lester the Farmer is an actor here, and a unique amulet quotes him: “The fever racks my body. It dances… rhythmless, endless! I fear there is but one remedy. MORE! I must hear more…”
Na-Krul has his own splinter line, four tiers, all resistance: Lesser +35, Splinter of Hellfire +250, Greater +375, Abyssal +625. Socketed it reads “You deal increased damage, but lose Critical Strike Chance.”
Five pieces, new this patch, every one a cosmetic: Head, Brisket, Foreshanks, Sirloin and Hooves of the Bovine. Helm, chest, gloves, pants, boots. Every class can wear all of it.
It is not a gameplay set. No set bonus in the game mentions these.
In the wardrobe every piece reads Earned as an in-game reward, which is the game’s own line for a drop. The shop, the battle pass, a season track, PvP, salvage and limited-time events each have a different line and none of them is used here. No store product, no battle pass tier, no vendor, no quest, no recipe, and an empty price field.
Of everything carrying the cow tag, one record is a container: Blessings of the Bovine, the mythic cache, “Grants several Moothic Unique items”, Torment only.
What is inside a cache is not in the files. Everything else has been ruled out.
New hidden achievement, Blessed by the Bait: “Slay the Lord of Relaxation.”
The Lord of Relaxation is Mefishto, a demonic fish set up for combat, which no other fish is.
One of the new hidden cube recipes takes ten gold and returns a Torn Net. Nothing in the files joins the two.
The cathedral is not a building in Tristram you can walk into. It is the first floor of a separate dungeon, Diablo - The Labyrinth, whose floors are named as they were in 1996: Cathedral: Level 2, Catacombs: Level 7, Caves: Level 10, Hell: Level 15 and 16.
What Ogden’s cap turns out to be is not in the files.
There is a separate actor called Gharbad the Strong, so letting him finish goes the way it always did.
